Founded over a decade ago, Avenue For Home Care has been dedicated to delivering quality health care services to individuals with special needs. Our Assisted Living Facility is customized to meet the needs of the residents that we serve. Our dedicated and skilled professionals are available 24 hours a day, 7 days a week to deliver the right care, and are committed to ensuring that the quality of care we provide contributes positively to the physical, psychological, and social well-being of our residents.
Avenue For Home Care is a stabilized Assisted Living Facility. We are licensed by the Minnesota Department of Health to provide Customized Living Services (Health-related and Supportive Services) to residents who live in a congregate (group) setting in Minneapolis and the surrounding areas.
Our professional care service starts with an individualized care plan detailing the required services to be provided to each resident. Our staff is trusted and respected in the industry, and we know fully well that the safety of our residents depends on the kind of employees we admit into our agency. We provide 24-hour customized living services for residents under age 65, as well as residents over age 65.
